DCH Receives $933,300 Grant from ISDH

Daviess Community Hospital has announced they’ve received a grant totaling $933,300 from the Indiana Department of Health.

The grant was awarded to establish a Community Paramedic Program as part of the state’s Health Issues and Challenges program, which was established by the Indiana General Assembly in 2021 with funding from the American Rescue Plan Act.

The Community Paramedic Program is a patient-centered model of care delivered in a patient’s home. This includes working with a team to coordinate in home visits as well as the use of telehealth. The Community Paramedic will assist with health screenings, vaccinations, treatments, and follow-ups.

The program will also include social work, substance abuse resources, mental health, chronic disease management, and more.

Daviess Community Hospital is one of more than 150 entities to receive funding for the Health Issues and Challenges program.

Priority was given to applicants that demonstrated high need and high impact in their grant proposals.

The state awarded more than $35 million statewide in the initial round of funding.
